How a facelift is performed
- Incisions begin in the temporal region, pass in front of or just inside the tragus, curve under the earlobe, and continue behind the ear into the hairline. Length depends on how much skin redundancy is present, particularly in the neck.
- The skin is elevated to a defined extent, the SMAS is treated by plication, excision, or sub-SMAS elevation depending on the technique selected, and the deeper layer is fixed under tension to stable fascia. The skin is then redraped along a separate vector, trimmed conservatively, and closed so that tension rests on the deeper layer rather than on the skin edges. Drains may be used.
