Why Upper Lid Results Appear to Fade
The most frequent cause is not the eyelid at all. As the brow descends, it pushes upper lid tissue downward and recreates hooding even when the eyelid skin excision was appropriate.
This is why a careful assessment separates brow position from eyelid skin. Removing more eyelid skin to compensate for a descending brow can tether the brow further and produce a heavy, closed appearance.
What Determines Individual Durability
- Brow position and whether it was addressed at the same time.
- Skin quality and sun exposure history.
- Genetic tendency toward periorbital fat prominence.
- Thyroid eye disease or other conditions affecting orbital tissue.
- Whether the initial excision was conservative, which is usually preferable but can shorten the perceived duration.
Limits of this answer
- Eyelid surgery does not stop periorbital aging. Crow's feet, pigment, tear trough deepening, and skin texture continue on their own trajectory and require separate treatment.
- Duration figures come from clinical experience and observational series rather than controlled long term studies, so they describe a pattern, not a guarantee.
- A patient whose real finding is brow descent will be dissatisfied with eyelid surgery no matter how well it is executed, and that misdiagnosis is the most common source of the belief that the surgery did not last.
