Short answer

Each has a different treatment, and treating the wrong one produces no visible change. Definition comes from matching the intervention to the finding, not from one signature procedure.

Profile Is Where This Gets Decided

The single most useful assessment is a true lateral photograph at rest. It reveals chin projection relative to the lips, the cervicomental angle, and whether the fullness sits above or below the jaw border.

Fullness above the jaw border is usually jowling from descent. Fullness below it is usually submental fat or a low hyoid position.

What Cannot Be Changed

  • Hyoid bone position.
  • Mandibular width and angle shape, short of skeletal surgery.
  • Submandibular gland position, which can create fullness that liposuction does not touch.
  • Skin quality, which determines how well the skin redrapes after fat is removed.

Limits of this answer

  • Liposuction of the neck in a patient with poor skin quality can worsen the appearance by revealing laxity that the fat was concealing.
  • Filler along the jaw border can improve definition modestly but cannot substitute for removing fat or repositioning tissue, and large volumes distort the lower face.
  • Anatomic ceilings are real. Some patients cannot achieve a sharply defined jawline at any level of intervention, and honest counseling about that is more useful than a staged sequence of procedures.
